After 14 years of
providing health care, Midlothian Orthopaedic Associates, Ltd. closed it's
doors at the end of March 2012.
Effective April 2012,
Dr. John York’s office will serve as
custodian of medical records for my former practice.
Dr. York’s office is located at at Bon
Secours St. Francis Medical Center which is reasonably close to the location
of my former practice at Johnston-Willis Medical Center. Dr. York may
also be pleased to take over your orthopedic care from myself, if you so
desire.
After March 31, 2012 the
patient’s records and imaging studies will have been transferred to Dr.
York’s office, so please submit any request(s) for copies of your medical
records and/or imaging studies to Dr. York’s office at the address/phone
number below.
